What Causes Wet Brain?
Wet brain is caused by a severe shortage of thiamine, a vitamin the brain needs to convert glucose into energy. Alcohol produces that shortage through 3 mechanisms at once:
- Heavy drinking inflames the gut and blocks thiamine absorption from food.
- Alcohol impairs the liver's ability to store and activate the vitamin.
- People in late-stage alcohol addiction often eat poorly, so little thiamine comes in at all.
Starved of thiamine, energy-hungry brain regions involved in memory, balance, and eye movement begin to fail. Alcohol does not have to be involved, severe malnutrition alone can cause it, but chronic heavy drinking is by far the most common path.

What Are the Two Stages of Wernicke-Korsakoff Syndrome?
Wernicke-Korsakoff syndrome unfolds in 2 stages: an acute, treatable emergency and a chronic, largely permanent condition.
Stage 1: What Is Wernicke Encephalopathy?
Wernicke encephalopathy is the acute stage and a genuine medical emergency. The classic warning signs are 3:
- Confusion and disorientation beyond ordinary intoxication.
- Loss of muscle coordination, especially an unsteady, wide-based walk.
- Abnormal eye movements, such as rapid jerking or trouble moving the eyes together.
Treated promptly with high-dose thiamine, usually given intravenously, Wernicke encephalopathy can improve substantially. Untreated, it can progress to the second stage or become life-threatening.
Stage 2: What Is Korsakoff Syndrome?
Korsakoff syndrome is the chronic stage that follows untreated Wernicke encephalopathy. Its signature is profound memory impairment: difficulty forming new memories, gaps in past memory, and confabulation, where the brain fills those gaps with invented details the person sincerely believes.
At this stage the damage is often permanent, which is why acting during stage 1 matters so much.
Can Wet Brain Be Reversed?
Wet brain can be partially or substantially reversed only when it is caught early. Thiamine replacement in the acute stage can resolve eye symptoms and improve coordination and confusion. Once Korsakoff syndrome is established, treatment shifts from cure to stabilization: stopping alcohol, restoring nutrition, and protecting the memory that remains.
The practical takeaway for families is simple. Confusion plus unsteadiness in a heavy drinker is an emergency room visit, not something to sleep off.
How Does Treatment Prevent Wet Brain?
Treatment prevents wet brain by ending the thiamine starvation at its source. Our medically supervised detox manages alcohol withdrawal with medical staff, nutritional support, and vitamin supplementation from day one, because withdrawal is precisely when a depleted brain is most vulnerable.
From there, our residential treatment program rebuilds physical health alongside the psychological work of recovery, with our clinical team monitoring cognitive recovery throughout.
When Should You Call for Help?
Call for help as soon as drinking has become daily and heavy, before any neurological symptoms appear. Wet brain is the late chapter of a story that treatment can interrupt in any earlier chapter, and the earlier the interruption, the more of the brain you keep.
Our admissions team answers 24/7 at (323) 709-7856. If you are seeing possible stage 1 symptoms right now, call 911 first, then call us about what comes after the hospital.
